ADDENDUM NO. 01
(For the procurement of 2 sets of Computerized Digital Radiographic Machine with complete accessories under Bid number B-11-0188 and Purchase Request Number 11-9950)
Please be informed that there are additional terms and conditions, after the pre-bid conference conducted yesterday, January 05, 2012, for the procurement of Purchase request number (PR) 11-9950 under bid number (11-0188) for the purchase of 2 sets of Computerized Digital Radiographic Machine with complete accessories, for use of various Hospitals of Compostela Valley, which public bidding was officially scheduled on January 17, 2012 @ 9:30 a. m.
Hereunder the following additional terms and conditions:
- Warranty period at least one (1) year
- Delivery site One (1) unit for CVPH-Maragusan and one (1) unit for CVPH-Montevista
- Delivery period is twenty (20) working days.
- After sales services shall be covered by supplier.
- Training modules should be supplied by the supplier and training of personnel shall be on site.
- UPS and AVR should be provided.
